Theander Pharmaceuticals is told to make an exception for prescription drugs containing sildenafil citrate, or generic Viagra (sildenafil). This would be when a prescription drug is not needed, but is a necessary one.
The exception will apply if the drug is already approved, but has a lower risk of serious side effects (eg, heart attack and stroke) compared to other medicines. The exception will not apply to Viagra (sildenafil).
Theander said the exception would apply to Viagra ‘unless the drug is approved for use in a particular population’, but that it could be prescribed in some special populations (eg, children).
‘We are aware of cases where the exception is not necessary or is not needed, but we are still investigating the possible impact of this exception on safety and quality of life,’ the company said.
‘If Viagra is prescribed in certain special populations (eg, children), it could lead to serious side effects that outweigh any benefits and could increase the risk of serious health problems.’
In response, Pfizer has taken a step in the right direction by publishing a safety summary (SYSV) for all Viagra generics. It is hoped this would reassure the public, as it is not clear whether the exception is necessary for all such generics.
In April 2013, the drug was approved for use in the UK for the treatment of erectile dysfunction (ED) and pulmonary arterial hypertension (PAH).
Sildenafil is the active ingredient in the branded Viagra, but it is not a treatment for PAH, and therefore the generic version is not yet available.
A spokesman for Teva Pharmaceuticals, which made the decision to sell generic Viagra (sildenafil), said: ‘The generic Viagra will not be available until November 2021, but patients may still be able to buy it.
‘Patients who are experiencing ED may still be able to benefit from taking the generic version of Viagra, although a separate safety and quality control application is currently in the works.
‘Sildenafil is intended to provide patients with a low likelihood of suffering serious adverse reactions, particularly heart problems,’ the spokesman added.
A separate safety summary was published in March 2014, which was updated to reflect the latest evidence.
SYSV contains information about the risk of heart attack, stroke and death in some other drugs, including the branded drug.
Theander Pharmaceuticals said it was reviewing the SYSV for ‘substantial evidence’ to inform patients of the risk of heart attack and stroke. Patients should only take the generic version of the drug if they are already taking other medicines.It added that it was ‘reviewing the available data to determine the safety and appropriateness of using the generic product’.
The SYSV covers a wide range of drugs, including the branded version of Viagra, but generic Viagra is available in a higher strength than the branded product, including the SYSV.
Teva Pharmaceuticals, which makes Viagra, said the new SYSV would give it the same information about the risk of heart problems, but would not include a mention of the generic product.
Teva Pharmaceuticals said it was ‘reviewing the available data to determine the safety and appropriateness of using the generic product’.Theander added that it had ‘an important role to play in developing and delivering innovative medicines, which provide patients with access to medicines with the potential to improve quality of life and reduce healthcare costs’.
Theander said that ‘there is an increased need to understand the risks associated with generics of some prescription medicines and that it is now planning to take a ‘black and white’ approach to its supply chain for all medicines.
